Título: Evidence of carcinogenic and inflammatory pathways induced by electronic cigarettes in the oral mucosa and head and neck

Resumo: Introduction: The increasing use of electronic cigarettes has emerged as a significant public health concern.
Objective: To evaluate evidence on vaping-induced cellular and tissue damage and to identify carcinogenic and inflammatory pathways associated with oral cancers.
Methods: A systematic review was conducted across PUBMED, Lilacs, SciELO, and Cochrane databases, including studies published between 2014 and 2024. Search terms comprised e-cigarettes, vaping, oral cancer, squamous carcinoma, and head and neck cancer. Fourteen original in vitro and in vivo studies assessing molecular and clinical alterations related to vape exposure were included. Risk of bias was evaluated (Ohat, Robins-E).
Results: Vaping exposure was associated with DNA damage, alterations in TP53, micronucleus formation, oxidative stress, and strand breaks. Elevated LDH levels and compromised membrane integrity suggest tissue injury and a permissive microenvironment for tumor development. Chronic inflammation emerged as a central mechanism, involving activation of ISG15-RAGE, NF-κB, and STAT3 pathways, along with increased TNF-α and interleukins, promoting proliferation, immune evasion, survival, migration, and epithelial-mesenchymal transition.
Conclusion: Electronic cigarettes activate carcinogenic and pro-inflammatory pathways in oral and head and neck tissues, promoting a microenvironment favorable to tumor initiation and progression. These findings underscore the need for standardized clinical studies and further investigation into compound-specific mechanisms.
