Resumo: Mucosal leishmaniasis is a severe form of tegumentary leishmaniasis and carries a risk of recurrence. While nasal involvement is the most common presentation, oral involvement is less frequent. We report two cases of oral mucosal involvement confirmed by PCR, both associated with therapeutic failure. Case 1: A 50-year-old woman with previously treated cutaneous leishmaniasis using intralesional therapy that progressed to the mucosal form. After treatment with meglumine antimoniate (Glucantime), she developed labial recurrence confirmed by PCR and subsequently received amphotericin B. Case 2: A 42-year-old man with cutaneous leishmaniasis treated with intralesional meglumine antimoniate in 2019, that progressed in 2025 with nasal involvement. After amphotericin B, he showed partial improvement but later presented lesion extension to oral mucosa, being treated with miltefosine. Oral involvement may represent a marker of greater severity. Histological and immunological analyses may contribute to understanding incomplete therapeutic response. Funding: CNPq, FAPEMIG and CAPES.
