Resumo: Clear cell odontogenic carcinoma (CCOC) is an infrequent odontogenic malignancy; approximately 82.1% of cases occur in the mandible, primarily in the molar-ramus area. We report a case of an 89-year-old woman presenting with an anterior mandibular tumor of 6 months duration, which affected a fixed prosthesis. Radiographic examination revealed a poorly defined radiolucent lesion with lingual and vestibular bone perforation. An excisional biopsy via curettage was performed. Histopathologically, multiple nests and epithelial cords of polygonal clear cells were observed invading the bone and fibrous connective tissue, alongside necrotic areas and scarce multinucleated giant cells. Immunohistochemical analysis was focally positive for AE1/AE3 and CK5/6, and diffusely positive for p40, p63, and EMA, with a Ki-67 index of 30%. The markers CK7, CK14, CK19, LCA, CK20, CD10, RCC, SOX10, S100, Synaptophysin, Chromogranin, Vimentin, and CD99 were all negative, supporting a diagnosis of CCOC. The patient is currently undergoing chemotherapy and radiotherapy.
