Resumo: Active dental infection control prior to hematopoietic cell transplantation (HCT) is important in reducing complications during neutropenia. The interval between dental surgical procedures and the start of conditioning for HCT is critical. This report describes a 55-year-old woman with diffuse large B-cell lymphoma refractory to two treatment regimens who developed sepsis during transplant-associated neutropenia related to a dental extraction site. Due to urgent transplantation, she underwent extraction of tooth 37 five days before conditioning for an active periapical abscess. On day +3, she developed grade 2 mucositis, febrile neutropenia, and septic shock. Intraoral examination revealed pain on palpation, pseudomembrane, alveolar debris, and erythematous margins without tissue coaptation, consistent with alveolar osteitis. Blood cultures grew Streptococcus salivarius. Management included hemodynamic stabilization, antibiotics, surgical debridement, chlorhexidine irrigation, and photobiomodulation. This case highlights the need for rigorous monitoring and the importance of discussing maintenance of antibiotic therapy until primary epithelialization during neutropenia.
