Resumo: Mucocutaneous leishmaniasis is a vector-borne parasitic infectious disease that may recur in the oral mucosa, particularly in systemically compromised patients. This study reports a case of a 76-year-old woman with type 2 diabetes mellitus, dialysis-dependent chronic kidney disease, and heart disease, previously treated for mucocutaneous leishmaniasis with liposomal amphotericin B and complete response. Following an acute myocardial infarction, she required intensive care unit admission with orotracheal intubation and mechanical ventilation, subsequently developing painful lesions in the oral cavity, nasal septum, and lower limbs. Intraoral examination revealed erosive-ulcerative granulomatous lesions affecting the vestibular sulcus, maxillary attached gingiva, and hard palate. Incisional biopsy confirmed leishmaniasis histopathologically. Due to comorbidities and progression despite amphotericin B, miltefosine was initiated, resulting in significant improvement. This case highlights the role of hospital dentistry in the diagnosis and management of recurrent mucosal infectious lesions in critically ill patients.
