Título: MEDICATION-RELATED OSTEONECROSIS OF THE JAWS ASSOCIATED WITH ORTHODONTIC APPLIANCE-INDUCED TRAUMA: A CASE REPORT

Resumo: Medication-related osteonecrosis of the jaws (MRONJ) is a multifactorial condition associated with antiresorptive therapy, generally trigged by invasive surgical procedures. A 42-year-old female was referred for evaluation of an exposed bone-healing on the hard palate. Her medical history included osteopenia, Hashimotos thyroiditis, prolonged oral bisphosphonate use, and previous smoking. Intraoral examination revealed a well-defined ulcer with bone exposure on the palatal surface of the maxillary tuberosity on the left side, a region where a removable orthodontic appliance was positioned. Periapical radiography showed no abnormalities, whereas cone beam computed tomography demonstrated a bone sequestrum, confirming MRONJ. Conservative management with topical chlorhexidine gel and adjustment of the orthodontic appliance resulted in spontaneous sequestrum exfoliation and complete mucosal healing. The patient remains under follow-up without recurrence. Dentists and, especially, orthodontists should be aware of the possibility of MRONJ occurring due to trauma from orthodontic appliances in patients who are taking bisphosphonates.
