Resumo: A 36-year-old male patient with Down syndrome presented with respiratory symptoms following household exposure to parents positive for SARS-CoV-2. Three days after symptom onset, he developed targetoid lesions on the lower limb and multiple ulcerative lesions affecting the dorsum, border, and ventral surface of the tongue, as well as the palate and labial mucosa, in addition to hemorrhagic crusts on the vermilion border. Comprehensive diagnostic investigation, including tests for autoantibodies, toxoplasmosis, HIV, herpes simplex virus (types 1 and 2), cytomegalovirus, Epstein-Barr virus, hepatitis B and C, Mycoplasma pneumoniae, syphilis, and cat-scratch disease, yielded negative results. After excluding other common etiologies, erythema multiforme was diagnosed, with prior COVID-19 infection identified as the sole triggering factor. Treatment consisted of low-level laser therapy (red wavelength) and topical 0.05% clobetasol gel, resulting in complete lesion remission.
