Resumo: A 73-year-old female patient presented with a complaint of swelling on the roof of her mouth, which had been present for approximately six months. Physical examination revealed an expansive lesion on the right side of the hard palate, respecting the midline, with a smooth surface and a color varying between pink and red. Imaging studies revealed an expansive, hypodense lesion involving the right nasal cavity. The diagnosis hypothesis was a benign salivary gland neoplasm. An incisional biopsy revealed a lesion composed of epithelial and myoepithelial cells, with the latter predominant, organized into sheets, blocks, nests, cords and duct-like structures. Immunohistochemical analysis showed that the neoplastic cells were diffusely positive for S-100 protein and focally positive for CK7, p63 and vimentin, with rare cells positive for SMA. The final diagnosis was myoepithelial carcinoma. The patient was referred for treatment of the lesion and is currently under follow-up with no recurrence.
