Resumo: A 31-year-old woman presented to the Oral Medicine Service complaining of tooth spacing associated with mandibular swelling with a 2-month history. Intraoral examination revealed buccal and lingual expansion in the lower incisors region, with a hard consistency, normal mucosal coloration, and marked tooth displacement. Radiographically, a mixed lesion was observed, predominantly radiopaque, with diffuse and irregular borders, associated with periosteal and root resorption. Computed tomography showed an intraosseous hyperdense lesion with radiating spicules, cortical expansion, and dental resorption. The diagnostic hypotheses included osteosarcoma, osteoblastoma, and chondrosarcoma. After an incisional biopsy, the diagnosis of chondroblastic osteosarcoma was confirmed. The patient underwent surgical excision with a lower limb graft performed by the Head and Neck Surgery team, without the need for chemotherapy, and remains under follow-up with no signs of recurrence or metastasis. Recognizing clinical and imaging features of this aggressive neoplasm and prompt investigation enable early diagnosis, minimizing metastasis and recurrence.
