Título: Oral manifestations of cytomegalovirus in a patient with pure red cell aplasia: a case report

Resumo: A 72-year-old male patient with heart failure, moderate mitral valve insufficiency, pure red cell aplasia, pulmonary cryptococcosis, under immunosuppressive oral anticoagulant therapy, presented with pancytopenia, dysphagia, and odynophagia. Intraoral examination revealed ulcerative lesions on lower lip vermilion, alveolar ridge, palate, along necrotic areas on the tongue and residual roots with mobility. Exfoliative cytology, performed due to bleeding risk, confirmed herpesvirus infection, polymerase chain reaction (PCR) testing indicated cytomegalovirus (CMV) infection. Treatment included systemic ganciclovir, six sessions of antimicrobial photodynamic therapy (aPDT) using 0.05% methylene blue associated with red low-level laser irradiation (9 J/cm²), seven sessions of photobiomodulation using red low-level laser (2 J/cm²), resulting in complete lesion remission. Immunosuppressed patients are more susceptible to opportunistic infections with oral manifestations. CMV typically presents as painful ulcers with erythema and necrosis. This case highlights the importance of dental professionals in diagnosing oral manifestations of systemic infections.
