Resumo: A 40-year-old white man presented with a painless, firm nodular swelling measuring 1.5 cm, located between the mandibular right canine and first premolar. The lesion had an estimated duration of 5 months, and pulp vitality tests of the involved teeth were positive. Cone-beam computed tomography revealed a multilocular radiolucent lesion associated with buccal and lingual cortical bone perforation and displacement of adjacent roots, suggesting an aggressive growth pattern. The main differential diagnoses included botryoid cyst, odontogenic keratocyst, and ameloblastoma. During incisional biopsy, aspiration yielded no fluid; however, solid tissue was obtained by curettage. Histopathological examination confirmed the diagnosis of solid ameloblastoma, and the patient underwent conservative surgical resection. At the 8-month follow-up, the patient showed good functional recovery, with no pain or paresthesia. Panoramic radiography demonstrated progressive bone healing in the surgical site. Due to the known aggressive biological behavior of this lesion, follow-up at 6-month intervals was recommended.
