Resumo: This case series included 45 patients diagnosed with proliferative verrucous leukoplakia (PVL) presenting clinical lichenoid features. The mean age was 59.9 years, with a strong female predominance (91.1%). Most patients were non-smokers (82.2%) and non-drinkers (86.7%). Clinically, the lesions were predominantly multifocal (100%), bilateral (88.9%), and frequently exhibited striated (75.6%) and plaque-like patterns. Homogeneous areas were less common (28.9%). Surgical management, mainly excisional biopsy, was the most commonly reported treatment. The mean follow-up period was 170.6 months. Malignant transformation occurred in 15.6% of cases, while recurrence was observed in 8.9%. PVL with lichenoid clinical features predominantly affects older women, often in non-smoking individuals, and presents as multifocal and bilateral lesions with a considerable risk of malignant transformation over long-term follow-up. Notably, lichenoid features may delay diagnosis, as they can mimic oral lichenoid lesions or oral lichen planus, underscoring the importance of longitudinal clinical monitoring of lesion evolution.
