Resumo: Allergic contact stomatitis (ACS) is an uncommon inflammatory reaction of the oral mucosa triggered by hypersensitivity to chemical agents present in oral care products, dental materials, foods, or medications. In July 2024, a new formulation of Colgate Total Clean Mint containing stannous fluoride (SnF₂) was introduced in Brazil, and emerging reports have described oral mucosal hypersensitivity temporally associated with its use. A 35-year-old woman who presented with painful ulcerative and whitish plaques affecting multiple intraoral sites, with significant impairment of eating and speaking, initially suggesting autoimmune disease. After incisional biopsy, microscopy demonstrated epithelial hyperplasia with spongiosis, exocytosis, and a lichenoid chronic inflammatory infiltrate. Immunohistochemistry showed a predominance of CD8+ T cells, numerous M2 macrophages, Langerhans cell hyperplasia, and, notably, presence of CD123+ plasmacytoid dendritic cells. Follow-up revealed recent initiation of a new SnF₂-containing toothpaste; discontinuation resulted in complete remission within a few days, confirming the diagnosis of ACS.
