Título: Diagnostic Performance and Practices of SUS Dentists in Identifying Potentially Malignant and Malignant Oral Lesions

Resumo: Objective: To evaluate the ability of dentists working in Brazil's Unified Health System (SUS) to classify oral lesions as benign, potentially malignant, or malignant, and to characterize their attitudes and clinical practices regarding oral cancer detection. Study Design: A cross-sectional study was conducted among public health dentists using a structured questionnaire to assess sociodemographic characteristics, attitudes, and clinical practices, followed by classification of 20 clinical cases of oral lesions. Sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) were calculated using a multiclass one-vs-all approach with 95% confidence intervals. Results: Thirty-two dentists participated; most were female (81.3%), 71.9% had over 20 years of clinical experience, and 90.6% graduated from Brazilian public universities. For malignant lesions, sensitivity, specificity, PPV, and NPV were 59.4%, 92.6%, 77.6%, and 84.2%, respectively. For potentially malignant lesions, the corresponding values were 56.3%, 75.0%, 54.8%, and 76.1%. When malignant and potentially malignant lesions were grouped and compared with benign lesions, sensitivity, specificity, PPV, and NPV were 72.7%, 77.2%, 82.5%, and 65.7%, respectively. Most participants reported insufficient training and limited comprehensive oral examinations in routine care. Conclusion: SUS dentists demonstrated limitations in oral cancer detection that may compromise early diagnosis.
