Resumo: Myoepithelioma is a rare benign neoplasm of the salivary glands, often posing diagnostic
challenges due to its morphological diversity. This report describes a case of a 39-year-old
woman presenting with a 0.7 cm painless, normochromic sessile nodule at the hard-soft
palate junction, with a three-year history. The initial clinical hypothesis was pleomorphic
adenoma. Excisional biopsy followed by histopathological examination revealed a benign
neoplasm composed of myoepithelial cells with diverse patterns, predominantly plasmacytoid
cells with eosinophilic cytoplasm and eccentric nuclei, arranged in islands within a hyalinized
stroma. Tumor cells were observed adjacent to minor salivary gland acini.
Immunohistochemistry demonstrated strong nuclear positivity for SOX10 and p63, alongside
cytoplasmic expression of S100 and vimentin. These findings were consistent with the
diagnosis of plasmacytoid myoepithelioma. This case reinforces that immunohistochemical
characterization is essential to address diagnostic challenges and ensure definitive diagnosis.
Following surgery, the patient remains with no evidence of recurrence.
