Resumo: Fanconi Anemia (FA) confers a high risk for oral squamous cell carcinoma, requiring rigorous diagnostic surveillance. We report three cases of FA patients (aged 27, 34, and 36) presenting chronic oral lesions, such as leukoplakia and Graft-versus-Host Disease manifestations, in high-risk sites (tongue, palate, and gum). Diagnosis was conducted using exfoliative cytology and incisional biopsies, confirming hyperkeratosis with varying degrees of epithelial dysplasia. Given the need for preventive interventions, management consisted of lesion ablation using a high-power surgical laser. The technique ensured excellent hemostatic control and favorable healing. Postoperative follow-up was performed through a hybrid protocol, combining in-person examinations and tele-dentistry for constant monitoring. In conclusion, surgical laser ablation is a safe and effective strategy, with lower rate of early recurrence proving fundamental for the conservative and preventive management of oral cancer in this ultra-high-risk group.
