Resumo: A 46-year-old female with multiple mandibular exostoses was under follow-up with a general dentist. During this period, progressive enlargement of a lesion previously interpreted as an exostosis was observed, prompting referral for specialized evaluation. Intraoral examination revealed bilateral mandibular tori on the lingual aspect and a firm nodular lesion on the buccal cortical plate in the region of teeth 32, 33, and 34. Computed tomography demonstrated a hypodense, apparently multilocular lesion associated with buccal cortical expansion. Incisional biopsy showed myxomatous connective tissue with abundant mucopolysaccharide-rich ground substance and sparse stellate cells with delicate cytoplasmic processes, consistent with odontogenic myxoma. The lesion was surgically excised with safety margins, and the diagnosis was confirmed. After 3 months, no recurrence was observed. This case highlights how anatomical variations may obscure pathological changes, delaying diagnosis and reinforcing the importance of reassessing lesions with unexpected clinical progression.
