Resumo: Central giant cell lesion (CGCL) is an uncommon benign bone lesion, more frequent in women and typically diagnosed in young individuals. This report describes a 36-year-old female patient who presented with localized pain in the right posterior mandible, associated with episodes of paresthesia and numbness in the right lower lip. Clinical examination revealed swelling, tenderness on palpation, and the presence of a yellowish pseudomembrane in the retromolar region. Computed tomography showed an extensive unilocular hypodense lesion with ill-defined borders, causing expansion and destruction of the cortical bone in the right posterior mandible. An incisional biopsy was performed, and histological analysis revealed connective tissue with inflammatory infiltrate, abundant multinucleated giant cells, and areas of interstitial hemorrhage, consistent with CGCG. A conservative approach was chosen, consisting of curettage associated with peripheral osteotomy. The patient has been under clinical and radiographic.
