Resumo: Invasive oral fungal infections in immunocompromised patients are associated with high morbidity, and early diagnosis is essential, typically based on ulceronecrotic lesions of the oral mucosa. This report describes early diagnosis prior to mucosal lesion development in the immediate pre-hematopoietic cell transplantation period. A 16-year-old patient with severe aplastic anemia underwent a second haploidentical transplantation after primary graft failure. Days before conditioning, he reported pain in the regions of teeth 25 and 26, without significant clinical and imaging findings. Local scraping was performed, and fungal culture identified Aspergillus spp. Due to transplantation urgency, conditioning was initiated with antifungal therapy. The condition progressed to a necrotic lesion with palatal bone exposure, without major systemic complications. After hematologic recovery, tooth extractions and endoscopic sinus surgery confirmed maxillary involvement without evident sinus extension. This case highlights the importance of early diagnostic investigation, even in the absence of evident lesions.
