Título: ORAL EXTRAMEDULLARY PLASMOCYTOMA IN A PATIENT WITH MULTIPLE MYELOMA: A CASE REPORT

Resumo: A 59-year-old male patient was referred to our unit with a prior diagnosis of Multiple Myeloma, undergoing oncological treatment under the "IREMA" protocol. After twelve therapeutic cycles, treatment was suspended due to tumor progression in the mouth. Intraoral examination revealed a tumor mass in the posterior region of the right mandible, measuring approximately 6 cm, firm and painful to palpation, with onset of approximately one month. Panoramic radiography did not show associated bone alterations. Laboratory tests demonstrated lymphopenia, thrombocytopenia, hypoalbuminemia, elevated serum urea levels, and C-reactive protein greater than 80 mg/L. An incisional biopsy was performed, and the histopathological report revealed a neoplasm composed of cells with hyperchromatic, oval and rounded nuclei, scant cytoplasm, intense nuclear pleomorphism, and frequent atypical mitoses, suggesting undifferentiated malignant neoplasm. Immunohistochemistry confirmed a plasma cell neoplasm with anaplastic morphology, consistent with Multiple Myeloma, exhibiting restriction in the kappa light chain.
