Título: PLASMACYTOMA MIMICKING AN ODONTOGENIC FISTULA: A CASE REPORT

Resumo: A 76-year-old female patient presented with a maxillary lesion clinically resembling a chronic periapical abscess, located between the roots of teeth 22 and 23. An excisional biopsy was performed, and histopathological examination revealed dense fibrous connective tissue with a predominant infiltrate of lymphocytes, macrophages, and plasma cells, without evidence of cellular atypia. For diagnostic confirmation, an immunohistochemical panel demonstrated positive expression for CD138 and lambda light chain, with negative expression for kappa light chain. Additionally, a low cellular proliferation index was observed, with Ki-67 estimated at 3%, supporting the diagnosis of solitary plasmacytoma. Subsequent oncological assessment and systemic investigations, including PET-CT, yielded negative results, confirming the diagnosis. The patient underwent surgical excision followed by adjuvant radiotherapy, with a satisfactory clinical outcome during follow-up.
