Título: ORAL ANGIOLEIOMYOMA: A DIAGNOSTIC CHALLENGE IN AN ATYPICAL LOCATION

Resumo: A 70-year-old male patient, melanoderma, non-smoker and non-drinker, was referred to the stomatology clinic with the complaint of "increased volume in my upper lip". Intraoral physical examination revealed an asymptomatic, ovoid, purplish, and fluctuant nodule, measuring 40 mm at its greatest diameter, located on the upper lip evolving over 18 months. The initial clinical hypotheses included lipoma and mucocele. Excisional biopsy was performed. Histopathological and immunohistochemistry analysis confirmed a diagnosis of Angioleiomyoma (AL). Histopathological examination revealed smooth muscle cells associated with blood vessels of varying sizes, often arranged in a perivascular pattern. The immunohistochemical study revealed strong staining for smooth muscle actin (SMA) and caldesmon. Angioleiomioma is a rare benign tumor in the oral cavity due to the scarcity of local smooth muscle, typically originating from the tunica media of blood vessel walls. Given its non-specific presentation, AL often mimics salivary gland tumors or reactive lesions like pyogenic granuloma.
