Resumo: An 81-year-old male patient, chronic smoker and with a previous history of lip cancer was referred for evaluation of multiple white verrucous plaques, asymptomatic and non-removable by scraping, located on the palate, tongue dorsum, and buccal mucosa. The diagnostic hypothesis was Proliferative Verrucous Leukoplakia. An incisional biopsy was performed, revealing hyperkeratosis associated with mild epithelial dysplasia, followed by lesion vaporization using high-power laser. Early recurrence was observed, along with the development of erythroplasic areas, whose histopathological analysis showed severe epithelial dysplasia. Management was challenging due to lesion extent, recurrent behavior, and limited therapeutic response, in addition to factors such as advanced age, poor treatment adherence, and continuous smoke habit. This case highlights the challenges in therapeutic decision-making in the context of aggressive lesion with high malignant transformation potential and associated clinical and contextual limitations.
