Resumo: Human Herpesvirus 6 (HHV-6) infection is associated with systemic and local outcomes; it is underreported in oral complications. We describe two cases of HHV-6-related oral infection in leukemic patients initially managed as mucositis but showing atypical features. Case 1: a 19-year-old male developed a firmly adherent whitish plaque on keratinized mucosa and lip edema on Day +5 after intrathecal methotrexate, progressing to multiple oral sites. Case 2: a 63-year-old female presented with ulcerated lesions with pseudomembranes on the hard palate on Day +5 after FLUBU, later extending to non-keratinized mucosa. The unusual onset on keratinized tissues, abrupt progression, and absence of mucositis in prior cycles prompted infectious investigation. Oral swab PCR showed high HHV-6 viral load; blood antigenemia was positive and PCR negative, suggesting recent hematogenous involvement. Both patients improved within 24 hours of antiviral therapy. These cases underscore the importance of recognizing atypical oral patterns in oncological patients.
