Título: Salivary and tear biomarkers in non-Sjögren sicca patients: a scoping review

Resumo: Objectives: To map and characterize the available evidence on salivary and lacrimal biomarkers in individuals with non-Sjögren sicca (nSS) highlighting gaps and potential biological mechanisms behind this condition.
Study design: This scoping review evaluated observational studies on salivary and lacrimal biomarkers in patients with nSS, in comparison with Sjögren disease (SjD). Extracted data included study characteristics, clinical variables, and biomarker-related information. Data were synthesized descriptively.
Results: Thirty-eight studies were included. Cytokines and chemokines (IL-6, IL-17A, TNF-α, CXCL10), and also β2-microglobulin, a component of MHC class I, were consistently lower in nSS than SjD in both saliva and tears. Lactoferrin levels rose in tears but dropped in saliva in nSS when compared to SjD. Salivary autoantibodies were detected in roughly 30-60% of nSS patients.
Conclusion: The analysis of biomarkers in tears and saliva serves as a dependable supplementary approach for distinguishing nSS from SjD. The overall reduction of inflammatory mediators argues against a fully established autoimmune process in nSS. However, autoantibodies positivity and mild inflammation may suggest nSS as an early or partial stage of SjD reinforcing the importance of careful monitoring these individuals.
