Resumo: Objective: To determine the frequency of oral potentially malignant disorders (OPMDs) and their association with demographic and behavioral risk factors in a prison population.
Study Design: Cross-sectional study conducted between August 2025 and January 2026 in a closed-subsystem prison in Valparaíso, Chile (total population: 3,564). A non-probabilistic convenience sample of 197 inmates was examined using structured clinical records and intraoral soft tissue examination. Diagnoses followed the 2021 WHO oral cancer classification. Inter-observer calibration was performed by an oral pathology specialist (minimum κ = 0.8). Bivariate association analyses were applied.
Results: OPMDs were identified in 14 of 197 participants (7.1%). Actinic cheilitis was the most frequent lesion (4.1%; n=8), followed by oral leukoplakia (3.0%; n=6). Frequency was higher in males (8.7%) than females (4.9%), without statistical significance (OR: 1.86; 95%CI: 0.56-6.14; p=0.310). Age group and alcohol consumption showed statistically significant associations with OPMD frequency (p=0.003 and p=0.043, respectively).
Conclusion: OPMDs were identified in approximately 1 in 14 examined inmates, with actinic cheilitis as the predominant lesion. Age and alcohol consumption were identified as significantly associated factors. These findings support routine oral mucosal screening in prison healthcare programs for early detection and prevention of oral cancer.
