Resumo: Oral leiomyoma is an uncommon benign neoplasm of smooth muscle differentiation that occurs predominantly in the lips and tongue. We report a case of a 63-year-old patient presenting with a nodule at the junction between lower left gingiva and buccal vestibule. The lesion was firm, white-yellowish, and measured 5 mm in diameter. Based on the provisional diagnoses of lymphoepithelial cyst and gingival cyst of the adult, an excisional biopsy was performed. Histopathological examination revealed a well-circumscribed proliferation of spindle cells arranged in a fascicular pattern, with positive immunohistochemical staining for SMA and HHF35, achieving the diagnosis of oral leiomyoma. A 3-month follow-up showed local recurrence, and a second excisional biopsy was performed. After the second surgical intervention, the patient remained disease-free for 2 months. This case highlights an unusual intraoral location and potential for recurrence in oral leiomyoma, underscoring the need for awareness of atypical presentations and careful follow-up.
