Título: CLINICAL PROFILE AND CONCORDANCE BETWEEN CLINICAL AND HISTOPATHOLOGICAL DIAGNOSES OF ORAL POTENTIALLY MALIGNANT DISORDER

Resumo: Objective: To evaluate the clinical profile and the concordance between clinical and histopathological diagnoses of oral potentially malignant disorders (OPMDs). Study Design: This is an epidemiological, retrospective, clinicopathological, and multicenter study, based on the analysis of secondary data from examination records and histopathological reports (2012-2023) from five Brazilian centers. Cases with a clinical diagnosis of OPMDs were included. Concordance was assessed considering the three main diagnostic hypotheses, using Pearsons chi-square test (p < 0,005) and the Kappa coefficient. Results: Among 22.303 cases, 1.124 (5,1%) corresponded to OPMDs, with a male predominance. Most cases were asymptomatic (86,3%), intraoral (77,2%), and presented as white lesions (79%) with a plaque-like appearance (80,3%). The tongue, lips, and buccal mucosa were the most affected sites. Leukoplakia was the main clinical diagnosis (73,6%), and epithelial dysplasia was observed in 76,6% of cases, predominantly mild. Overall concordance was 93% (Kappa = 0,839), being higher in white lesions, actinic cheilitis, and red lesions, and lower in lichenoid lesions. Conclusion: Despite the high overall clinical accuracy, histopathological evaluation remains essential for definitive diagnosis and for strengthening early diagnostic strategies.
