Resumo: A 25-year-old Bolivian male patient presented with a one-year history of tongue discomfort. Intraoral examination revealed multiple well-defined sessile nodules with a smooth surface, soft consistency, and coloration similar to the mucosa, bilaterally distributed on the buccal mucosa, borders of the tongue, and lower labial mucosa. Based on the clinical hypothesis of focal epithelial hyperplasia, an excisional biopsy of a lesion on the right tongue was performed. Histopathological analysis showed parakeratinized stratified squamous epithelium with a wavy surface, marked acanthosis, and elongated epithelial ridges in a "golf club" pattern, along with koilocytotic cells and mitosoid figures. The final diagnosis was focal epithelial hyperplasia (Hecks disease). The patient was referred for antimicrobial photodynamic therapy and remains under follow-up. This case highlights the importance of recognizing this condition which is endemic in Latin America (Amazon region), as a low-morbidity lesion that may cause aesthetic concerns or discomfort due to repeated trauma.
