Título: Mucus extravasation phenomenon ultrastructural characterization of foamy macrophages: A view beyond light microscopy

Resumo: Oral mucoceles are benign lesions arising from minor salivary glands, most frequently affecting the labial mucosa. They typically result from rupture of the glandular duct epithelium, leading to mucus extravasation into the surrounding connective tissue and subsequent pseudocyst formation (extravasation mucocele). Due to the limited ultrastructural data regarding macrophages involved in this lesion, this study aimed to evaluate the ultrastructural features of foamy macrophages in an oral mucus extravasation lesion, specifically a lower lip mucocele in a young patient without a clearly identifiable etiological factor. Surgical excision was uneventful, and the lesion was completely removed, revealing a well-circumscribed oval structure with gelatinous content. Tissue samples were fixed in 2% glutaraldehyde (0.1 M sodium cacodylate buffer), post-fixed in osmium tetroxide, dehydrated in graded acetone, and embedded in epoxy resin prior to polymerization at 60 °C for transmission electron microscopy (TEM) analysis. Additional samples were fixed in Carson's formalin, paraffin-embedded, and stained with hematoxylin and eosin for light microscopy. Ultrastructural analysis highlighted foamy macrophages with intense mucin phagocytosis and intracellular accumulation, with electron-lucent inclusions consistent with mucopolysaccharides, providing insights into their role in the inflammatory microenvironment.
