Resumo: Polymorphous adenocarcinoma (PAC) is a malignant tumor that primarily affects the palate and originates in the minor salivary glands. Due to its indolent presentation, PAC can pose diagnostic challenges. Here, we present the case of an 81-year-old patient with a one-year history of an asymptomatic palatal mass. Clinical examination revealed a purplish nodule measuring approximately 3 cm with surface ulceration and associated reactive lymphadenopathy. The differential diagnosis included mucoepidermoid carcinoma, pleomorphic adenoma, lymphoma, and adenocarcinoma. An incisional biopsy was performed under local anesthesia without complication. Histopathological findings suggested adenocarcinoma, prompting an immunohistochemical analysis to confirm the diagnosis. Postoperative follow-up revealed satisfactory healing. However, the lesion persisted with an ulcerated, inflammatory appearance. The patient remains under close clinical surveillance pending a definitive diagnosis. This case highlights the importance of combining histopathology with immunohistochemistry to accurately diagnose palatal lesions.
