Resumo: A 72 year old patient, diagnosed with multiple myeloma in 2008, presented with a two-year history of oral discomfort. The patient had a history of denosumab use for four years and was currently being treated with lenalidomide and daratumumab. Clinical and radiographic examination revealed bone exposure in the maxilla, in the region of teeth 13 to 15, consistent with drug-related osteonecrosis of the jaws.Management included combination therapy with pentoxifylline and tocopherol (PENTO protocol), combined with 60 sessions of hyperbaric oxygen therapy. During follow-up, clinical improvement was observed, with elimination of the necrotic bone and resolution of symptoms.The patient remains under clinical and radiographic follow-up, with stable condition and no progression. The therapeutic combination used demonstrates potential as an adjuvant approach in refractory cases, reinforcing its relevance.
