Resumo: The aneurysmal bone cyst (ABC), is a cyst/multicystic osteolytic neoplasm, with an incidence of 0.14 cases per 100,000 individuals, of which 1.5% ocurrs in the jaws. A case of 14 years old female patient was received for re-evaluation and histopathological analysis with a previous diagnosis of central giant cell granuloma. The macroscopic specimen showed on its cutting surface an aneurysmal macroscopic pattern; histologically, a fibrovascular neoplasm, composed of sinusoidal spaces filled with erythrocytes, separated by fibrous septa, with osteoclast-like giant cells, immature bone trabeculae, and osteoid material in a "lace-like" pattern was observed. Based on the histopathological findings and their correlation with the previous assessment, laboratory studies, and the provided tomographic and radiographic images, a final diagnosis of ABC was established. This case reinforces the importance of clinicopathological integration, particularly in lesions with overlapping features, where accurate diagnosis may require multidisciplinary evaluation.
