Título: EARLY DIAGNOSIS OF TONGUE SQUAMOUS CELL CARCINOMA IN A PATIENT WITHOUT RISK FACTORS: A CASE REPORT

Resumo: A 47-year-old non-smoking, non-drinking female patient presented with tongue pain,
attributed to a third molar. She was referred for oral medicine evaluation by a private
dentist despite two prior biopsies, one inconclusive and another suggestive of
pyogenic granuloma. Intraoral examination revealed a heterogeneous leukoplakia
with erythematous and granulomatous areas on the left border of the tongue,
measuring approximately 4 cm. Incisional biopsies were performed in two distinct
areas. Histopathological analysis demonstrated a malignant neoplasm characterized
by proliferation of atypical keratinocytes with marked cellular and nuclear
pleomorphism, along with small epithelial islands invading the lamina propria and
dense connective tissue with intense mononuclear inflammatory infiltrate, consistent
with squamous cell carcinoma. The patient was referred for oncologic treatment.
This case underscores the importance of maintaining clinical suspicion when findings
are inconsistent, even in atypical cases, like the absence of classical risk factors, in
order to achieve an early diagnosis of oral cancer.
