Título: Extrafollicular Adenomatoid Odontogenic Tumor Presenting as a Periapical Lesion in the Posterior Maxilla

Resumo: Extrafollicular adenomatoid odontogenic tumors (AOTs) in interradicular or periapical
regions of posterior teeth are rare and may mimic inflammatory periapical lesions. We
report an unusual case of an 18-year-old female with an asymptomatic swelling in the
posterior maxilla of unknown duration. Imaging showed a well-circumscribed, unilocular
radiolucency (2 cm) in the periapical region of the left maxillary second premolar. The
tooth exhibited external root resorption but remained vital. Buccal cortical expansion,
focal cortical plate destruction, and lingual tooth displacement were observed.
Excisional biopsy was performed with tooth preservation. Histopathology revealed a
well-circumscribed proliferation of odontogenic epithelial cells arranged in cords and
nests, with duct-like and rosette-like structures and focal calcifications in a fibrous
stroma. These findings supported the diagnosis of an extrafollicular AOT. No
recurrence was observed after one year. Clinicians should consider AOT in the
differential diagnosis of periapical radiolucencies in the maxillary premolar region.
