Título: Screening Bone Disorder in CKD Using Mandibular Radiomorphometric Indices: Retrospective Study with DXA and Biomarkers

Resumo: Introduction: Chronic kidney disease (CKD) impairs bone metabolism and may reduce bone mineral density (BMD). Although Dual-energy X-ray absorptiometry (DXA) is the gold standard, its limited accessibility highlights the need for alternative screening methods, such as mandibular radiomorphometric indices (RIs) assessed on panoramic radiographs (PRs) for bone mineral disorder detection. Objective: To evaluate RIs in patients with CKD using DXA-derived BMD and biochemical markers as reference parameters. Study Design: A retrospective observational study was conducted using 224 PRs of patients with CKD (stages III, IV, and V). The Mandibular Cortical Index (MCI) and Cortical Index of the Mandibular Canal (CIMC) were analyzed. The results were compared with serum levels from blood tests and the DXA results. Results: Most patients were female (56.6%), with a mean age of 48 years. MCI was not associated with osteoporosis by DXA (p=0.44), whereas CIMC showed a significant association, with altered values increasing the odds of osteoporosis by 87% (p=0.07). No significant differences were observed in biochemical markers among groups (p>0.05). Conclusion: CIMC may serve as a potential screening tool for osteoporosis in CKD patients, showing association with DXA-derived BMD, whereas MCI were not associated.
