Título: RECURRENT ORAL ULCERATIONS IN AN ELDERLY PATIENT: A DIAGNOSTIC CHALLENGE

Resumo: An 83-year-old woman presented with painful ulcerations affecting buccal, labial, and ventral tongue mucosae, with a similar episode occurring one month prior. Given the symmetrical distribution and patient age, a presumptive diagnosis of recurrent Varicella-Zoster Virus or Herpes Simplex Virus infection was considered. Serological testing for viral infections revealed positive IgG and IgM for Cytomegalovirus (CMV). Treatment included antimicrobial photodynamic therapy using methylene blue and red laser (initially 6 J/point, 3 J/point during healing, and 9 J/point in the last recurrence), combined with benzydamine, clobetasol, and chlorhexidine, resulting in remission within seven days. After 21 days, a new recurrence occurred on the contralateral side, and the same protocol was applied. CMV (human herpesvirus 5) is a prevalent virus that remains latent and may reactivate under immunosenescence. This case highlights an atypical presentation in an immunocompetent patient and suggests potential underdiagnosis due to the clinical similarity with more common viral infections.
