Título: Extensive Cemento-Ossifying Fibroma of the Mandible: A Management Challenge

Resumo: Cemento-ossifying fibroma (COF) is a benign fibro-osseous neoplasm that typically presents as a slow-growing swelling in the posterior mandible of adult women. A 25-year-old female presented with feeding and speech difficulties due to a firm swelling in the floor of the mouth, noted three years earlier. Imaging exams revealed a large (~5 cm), mixed radiolucent-radiopaque lesion in the anterior mandible with bilateral posterior extension, causing marked expansion and downward bowing of the mandibular inferior border. Incisional biopsy was performed, and microscopical examination revealed a fibrous stroma with bone trabeculae and cementum-like calcifications. Correlation of clinical, radiographic, and microscopic findings established the diagnosis of COF. Due to the lesion's extent, initial osteoplasty with tooth preservation was performed, and the patient remains under close follow-up pending definitive enucleation. This case highlights the potential for significant growth of COF, leading to functional impairment and posing challenges for clinical management.
