Resumo: A 75-year-old female patient presented with a persistent non-healing white lesion on the left mandibular alveolar ridge of 20 days duration. Clinical examination revealed a non-homogeneous leukoplakic lesion with erythematous component (leukoerythroplakia). The patient had a history of tongue squamous cell carcinoma in complete remission for eight years. Optical fluorescence imaging using a violet LED device (E-light®, 410 nm) was performed prior to incisional biopsy. The fluorescence examination demonstrated a heterogeneous pattern with intense red fluorescence centrally, consistent with porphyrin accumulation and reduced autofluorescence indicating stromal collagen degradation. Histopathological examination revealed moderately differentiated, minimally invasive squamous cell carcinoma with atypical epithelial cell nests infiltrating the underlying connective tissue.This case demonstrates the clinical value of optical fluorescence imaging as an adjunctive tool for identifying high-risk areas and directing targeted biopsy site selection in potentially malignant oral lesions. Integration of optical fluorescence imaging into multimodal diagnostic protocols enhances early oral cancer detection.
