Título: DELAY ACROSS THE ORAL CANCER CARE PATHWAY: A SYSTEMATIC REVIEW

Resumo: Objective: To identify and synthesize barriers associated with diagnostic and treatment delays in oral cancer across different income settings. Study design: Systematic review conducted according to PRISMA guidelines and registered in PROSPERO. Five databases were searched without restrictions. Observational, qualitative, and mixed-methods studies evaluating barriers related to delay were included. Barriers were standardized and mapped to Aarhus diagnostic intervals. Narrative synthesis was performed. Results: Sixteen studies (n=2,339 participants) from nine countries were included. Barriers were grouped into behavioural, sociocultural, economic, geographic, organizational, and provider-related domains. In high-income countries, delays were mainly associated with appraisal and help-seeking barriers, including symptom misinterpretation and psychosocial factors. In low- and middle-income countries, barriers extended across multiple intervals, including limited health literacy, use of traditional practices, restricted access to services, misdiagnosis, and delayed referral. Professional and organizational barriers became more prominent in diagnostic and pre-treatment intervals. Conclusion: Diagnostic and treatment delays in oral cancer represent a multifactorial and cumulative process across care pathways. Differences between settings reflect variations in trajectory complexity rather than distinct mechanisms. Interventions should target multiple levels, including patient awareness, professional training, and healthcare system organization.
