Título: Morphometric alterations of the mandibular condyles in people living with HIV

Resumo: This study evaluated alterations in trabecular bone microarchitecture of the mandibular condyle in people living with HIV (PLHIV) undergoing Highly Active Antiretroviral Therapy(HAART) compared to healthy controls. A total of 132 individuals were included, consisting of 40 PLHIV and 92 control subjects, all of whom underwent computed tomography imaging. Thirty-six morphometric parameters were assessed using CT Analyser software. The results showed that most parameters were preserved between groups. However, two variables demonstrated statistically significant differences: the number of closed pores(p<0.001) and the surface area of closed pores(p<0.041), both of which were lower in the PLHIV group. These findings suggest localized changes in bone microstructure. Correlation analysis revealed similar overall patterns between groups, with a high number of strong correlations in both, such as Total Volume of Pore Space vs Bone Surface Density(p=0.001, R=-0.441). Nevertheless, differences in correlation strength indicated subtle alterations in trabecular organization and possible compensatory mechanisms in PLHIV.
In conclusion, HIV infection and the HAART is associated with selective and measurable changes in the mandibular condyle's trabecular bone microarchitecture. Although most structural parameters remain unchanged, specific modifications highlight the potential of this anatomical site as a complementary region for assessing bone health in individuals living with HIV.
