Resumo: Lymphatic malformations (LMs) are vascular anomalies that most commonly affect the tongue within the oral cavity; involvement of the buccal mucosa is uncommon and may pose diagnostic challenges. A 17-year-old male presented with a five-year history of a submucosal nodule on the left buccal mucosa associated with repeated local trauma. Clinical examination revealed a lesion with coloration similar to the adjacent mucosa, focal brownish areas, and a circumscribed region exhibiting translucent vesicle-like structures consistent with a "frog spawn" appearance. Due to the atypical location and trauma history, incisional biopsy was performed. Histopathological analysis revealed numerous dilated lymphatic channels containing eosinophilic proteinaceous material with identifiable valve-like structures, confirming LM. Complete excision was performed using high-power laser, with no recurrence reported at follow-up. This case highlights that recognition of characteristic clinical features, particularly the "frog spawn" appearance, is essential for diagnosis even in atypical intraoral sites.
