Título: Rapidly progressive invasive oral aspergillosis in a patient with acute lymphoblastic leukemia receiving palliative care

Resumo: A 39-year-old male with acute lymphoblastic leukemia (ALL) in bone marrow aplasia was admitted to palliative care due to refractory oncogical treatment. He reported an oral lesion; intraoral examination revealed an asymptomatic dark macule on the left hard palate. Due to thrombocytopenia, exfoliative cytology was performed, initially suggesting Candida albicans. The lesion progressed to ulceration with tissue invasion, necrosis, and slight mobility of adjacent teeth. After multidisciplinary discussion, debridement and biopsy were performed under blood transfusion. Histopathology showed acute necrotizing inflammation with fungal colonies consistent with Aspergillus sp., confirming invasive oral aspergillosis. Photodynamic therapy (0.01% methylene blue, 660 nm diode laser, 100mW, 9 J/point) combined with systemic antifungals provided partial infection control; however, disease progression led to oroantral communication and death. This case highlights the importance of dental care in immunocompromised patients, where early diagnosis of invasive fungal infections is critical for prognosis.
