Título: CONSERVATIVE MANAGEMENT OF MANDIBULAR DESMOPLASTIC FIBROMA IN A PEDIATRIC PATIENT: A CASE REPORT WITH 9-YEAR FOLLOW-UP

Resumo: Desmoplastic fibroma (DF) is a rare, benign yet locally aggressive primary bone neoplasm. A 6-year-old male presented with swelling in the posterior mandibular region, noted over a two-month period, which was asymptomatic and associated with an unremarkable medical history. Intraoral examination revealed firm buccal and lingual swelling in the retromolar area covered by normal mucosa. Panoramic radiography showed involvement of the mandibular body, angle, and ramus, beneath the developing tooth germ of 37. Computed tomography demonstrated a poorly defined hypodense area (25×24 mm) with lingual cortical resorption and lateral cortical thinning. An incisional biopsy was performed, followed by complete surgical removal and placement of a 2.4-mm titanium plate. Histopathological and immunohistochemical analyses for HHF-35, α-SMA, and S-100 confirmed DF. Nine years of follow-up demonstrated that conservative surgery was effective, preserving anatomical structures and minimizing morbidity in this young patient.
