Título: PRIMARY ORAL MANIFESTATION OF VISCERAL LEISHMANIASIS IN A PATIENT WITH MYELODYSPLASTIC SYNDROME

Resumo: A 74-year-old man, brown, with hypertension and diabetes, was referred to our service for evaluation of oral lesions. The patient was diagnosed with myelodysplastic syndrome 2 years ago, and currently remains under therapy with recombinant human erythropoietin 4000 IU SC (rHu-EPO). Extraoral examination revealed crusted, ulcerated lesions on the face, right earlobe, nasal mucosa, and lips. Intraoral examination displayed painful ulcers on the tongue, and on the upper and lower labial mucosa. Paranasal sinus CT showed no abnormalities. Blood count revealed severe leukocytosis (40.460 cells/µL); however, myelogram and immunophenotyping excluded the transformation of myelodysplastic syndrome to acute myeloid leukemia. Histopathological examination of the lower labial mucosa revealed the presence of Leishmania sp. inside macrophages. IgG serological test was reactive confirming human visceral Leishmaniasis. Complementary tests ruled out the occurrence of disease in other regions. The patient showed resolution of lesions after treatment with Liposomal Amphotericin B.
