Título: Metastatic Prostate Adenocarcinoma to the Mandible with Sunburst Periosteal Reaction

Resumo: We report an unusual case of metastatic prostate adenocarcinoma to the posterior mandible in an 81-year-old male presenting with abdominal distension. Intraoral examination revealed a 1.5 cm firm, asymptomatic bluish swelling in the right posterior alveolar ridge with a six-month history, associated with lower lip paresthesia. Imaging demonstrated an extensive multilocular radiolucency with cortical destruction and a prominent sunburst-type periosteal reaction. An initial clinical diagnosis of osteosarcoma was considered, and an incisional biopsy was performed. Histopathological analysis showed nests of pleomorphic epithelial cells with glandular differentiation, immunopositive for prostate-specific antigen (PSA), confirming metastatic prostate adenocarcinoma. Notably, the patient was previously unaware of the primary malignancy, with markedly elevated serum PSA levels (3755 ng/mL). The patient was referred for oncologic management and is currently undergoing chemotherapy under multidisciplinary care, including palliative support. This case highlights an aggressive clinical-radiographic presentation of mandibular metastasis as the first manifestation of an undiagnosed prostate carcinoma.
