Resumo: This case series describes female patients presenting oral manifestations corresponding to different stages of syphilis. A 30-year-old patient presented with chancre-like oral lesions and a prior history of genital chancre and lymphadenopathy; rapid testing and VDRL confirmed syphilis. Another 30-year-old woman exhibited an asymptomatic whitish nodular lesion on the lower lip (1 cm), while a 17-year-old patient presented mucous patches on the posterior tongue and erythematous palatal plaques; both cases were confirmed by reactive rapid testing and VDRL. A 70-year-old woman with intellectual and visual impairment presented palatal perforation consistent with tertiary syphilis; initial non-treponemal testing was non-reactive, with diagnosis confirmed by treponemal testing, suggesting a prozone phenomenon. All patients were referred for specialized infectious disease management. These cases highlight the clinical variability of syphilis and the importance of recognizing diagnostic pitfalls.
