Resumo: Lichenoid and granulomatous stomatitis (LGS), was firstly described in 2006, reporting 6 cases, preferentially affecting the upper lip and adjacent gingiva of adult females, and whose clinicopathological features were similar with lichenoid and granulomatous dermatitis. To date, about 65 cases of LGS have been reported. Clinically, LGS may mimic oral lichen planus, oral lichenoid lesion, oral leukoplakia, squamous cell carcinoma, vesiculobullous lesions, trauma-associated and allergy lesions. Microscopy shows three distinct components, including lichenoid inflammation, granulomatous inflammation, and lymphoid follicles/aggregates in perivascular and perineural distribution. A 75-year-old woman was referred to presenting ulcerative lesions on the lower lip and adjacent gingiva, and buccal mucosa, several months ago. Microscopy evidenced a lichenoid mucositis associated with diffuse macrophage infiltration and numerous lymphoid follicles located in the lamina propria and submucosa. Serological and histochemical analysis excluded the possibility of infectious disease, and anti-Treponema pallidum was negative. After treatment with topical steroids, the lesions resolved.
