Resumo: Glomangiopericytoma is a rare mesenchymal sinonasal neoplasm with perivascular myoid differentiation. Two cases are reported. Case 1: a 73-year-old male with chronic epistaxis and nasal congestion, previously diagnosed as hemangiopericytoma at an outside institution. Case 2: an 80-year-old male with left facial swelling and an incidentally identified sinonasal mass. Three years after endoscopic removal, he returned with recurrent epistaxis and a polypoid nasal mass observed on CT, subsequently treated with functional endoscopic sinonasal surgery. Histologic examination of both cases showed a diffuse syncytial proliferation of ovoid to spindled cells with interspersed vascular channels. Immunohistochemistry demonstrated positivity for beta-catenin, CD34, cyclin D1, LEF1, and SMA, with negativity for STAT6, S100, and SOX10, confirming the diagnosis of glomangiopericytoma. Classified by the World Health Organization as a neoplasm of low malignant potential, accurate diagnosis is essential to distinguish this entity from other sinonasal spindle cell neoplasms and guide appropriate treatment.
