Resumo: A 39-year-old female patient developed vaginal candidiasis and initiated oral fluconazole 150 mg on a weekly regimen. Approximately two hours after administration, she presented with lip edema, oral pain, dysphagia, and abdominal cutaneous lesion. Clinical examination revealed ulcerative oral lesions and cutaneous findings consistent with erythema multiforme. The temporal association between fluconazole use and the onset of symptoms, together with recurrence following re-exposure to the drug 40 days later, allowed the establishment of a final diagnosis of fluconazole-induced erythema multiforme. Initially, corticosteroids, antihistamines, and analgesics were prescribed. As an adjunctive therapy, antimicrobial photodynamic therapy was instituted in combination with simultaneous low-level red and infrared laser irradiation (660/808 nm, 200 mW, 4 J/cm² per point, 20 s), following prior application of 0.01% methylene blue for 5 minutes. Five daily sessions resulted in significant improvement from the second application onward. During recurrence four sessions controlled the condition without lesion progression.
